
Jasmin Kilby ~ PCN Pharmacist Days of work: Monday and Wednesday at Barton House Medical Practice
Studied at Portsmouth University, completed a year post-graduate training with Boots and also became a Boots Macmillan Pharmacist and Dementia Friend. Worked in community pharmacy for 3 years and then worked for Somerset CCG/ICB helping GP surgeries with reaching their QoF targets
Jasmin works within Jurassic Coast PCN but is available for face to face or telephone consultation at the surgery on Wednesdays or Thursday.
Jasmin is able to offer the following:
- Medication reviews medication,
- Support GPs and other healthcare staff where possible.
- Provide support to patient’s regarding their medication and signpost them to other services provided within the NHS.
- Structured Medication reviews
- Reviewing your medication if you have been taking medication over a long time
- Discuss changes in medication, either after a stay in hospital or seeing a consultant
- Queries about medications
- Experiencing side effects from medications
- Managing a patient’s condition holistically
- Ensuring patients get the most out of their medication
- Ensuring there is joined-up care for patients.
Jasmin is currently working towards beaming an Independent Prescribing and diabetic specialist.

Having a clinical pharmacist at our practice means that you can be treated by the best professionals for your needs. The role is pivotal to improving the quality of care, ensuring patient safety, outcomes and value through a person-centred approach.
Having clinical pharmacists in GP practices means that GPs can focus their skill where they are most needed, for example on diagnosing and treating patients with more complex conditions.
