Register with us as a New Patient

New Patient Registration

Come to the surgery and ask for a new patient pack or register online. You should inform reception if you have a preference to see a particular doctor, and this will be recorded in your notes.

Please provide proof of identity and address when returning the form. We are unable to book an appointment for you until we have received this.

  • All new patients are given a registered GP but all our patients can see a GP of their choice
  • Please note that this practice will not tolerate violent or abusive patients. Such patients will be removed from the practice list

  • Medical treatment is available from the date of registration. Please contact reception for further information
  • Please note that your medical records may take several weeks to arrive at the Practice

Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).

There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.

The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
